Wellness Care Program Terms and Conditions:
- Additional charges will apply for nursing procedure conducted during nurse caregiving.
- For appointment escort services, transport fees are not included and are to be borne by the patients.
- The advance credit purchased must be utilized within 30 days from the date of purchase. Any unused credits will be forfeited, and no refunds or carryforwards will be provided.
